Pyridoxine in acute isoniazid overdose.

چکیده
Isoniazid is the mainstay of antitubercular therapy and is included in nearly every drug regime. Cases of intentional and accidental overdose have been reported(l,2) though with less than expected frequency considering the drug's extensive use. The principal manifestations of isoniazid overdose include recurrent seizures, metabolic acidosis and coma(3,4). We report two cases of isoniazid overdose and their successful management with high dose oral pyridoxine.

Isoniazid Toxicity
Pharmacokinetic characteristics of INH are presented. Adverse effects and toxicity caused by INH has been reviewed under the headings of neuropathy and seizures, hepatotoxicity, rhabdomyolysis, agranulocytosis, acidosis and other adverse drug reactions. Cases of poisoning by INH have been mentioned independently. Few case reports indicating symptoms of intoxication and the mechanism of toxicity...
متن کاملIsoniazid: a review with emphasis on adverse effects.
A review of the adverse effects of isoniazid (INH) wm undertaken and it was found that these effects could be divided into: (1) toxic, (2) idiosyncratic and (3) hypersensitivity reactions. Major toxic reactions are peripheral neuropathies at conventional dosage and seizures from overdosage. Pyridoxine is of benefit in these disorders. Other presumed toxic reactions are autonomic neuropathy, enc...
متن کاملPyridoxine deficiency in children treated with isoniazid.
Isoniazid-induced deficiency of pyridoxine (vitamin B6) is reportedly not uncommon in adults but rare in children. In the present study, 38 children had serum levels of pyridoxine tested while receiving therapy with isoniazid. A biologic assay using the protozoan Tetrahymena thermophila determined pyridoxine status after 2 to 18 months of therapy with isoniazid. Five children (13 percent) were ...
متن کاملRhabdomyolysis due to isoniazid poisoning resulting from the use of intramuscular pyridoxine.
Isoniazid is an effective antituberculosis drug. Isoniazid poisoning produces a characteristic clinical syndrome that occurs 30 to 120 minutes after ingestion and includes seizures, metabolic acidosis, and in severe cases, coma. Rhabdomyolysis is one of the reported complications of isoniazid poisoning, but relevant data are limited. Parenteral pyridoxine is the antidote of isoniazid. In this c...
متن کاملChronic dialysis, NAT2 polymorphisms, and the risk of isoniazid-induced encephalopathy – case report and literature review
BACKGROUND Isoniazid is the most widely used anti-tuberculosis agent, yet it may lead to life-threatening complications. CASE PRESENTATION Here we report the case of a chronic hemodialysis patient who developed severe encephalopathy after the start of isoniazid. Blood levels of isoniazid were elevated, and acetyl-isoniazid over isoniazid ratio was decreased 3 h after intake of the medication,...
